Administration of medication at school

In accordance with the Department of Education’s Administration of medication in schools procedure, schools require medical authorisation to administer any medication to students (including over-the-counter medications such as paracetamol or alternative medicines).

If your child is in need of routine medication, or may be in need of emergency medication, please complete the appropriate form below and return to the school with your child’s medication. Please note that all medication must be in the original medication container with a valid pharmacy label which includes the name of the prescribing health practitioner, your child’s full name and dispensing instructions.

If your child has a significant medical condition that may also require an individual Health Action Plan, please make contact with your child’s Head of House to discuss.
